<p class="wp-block-paragraph"><strong>September 2026 is Pulmonary Fibrosis Awareness Month</strong>, an opportunity to increase awareness about pulmonary fibrosis and help patients and families better understand this group of lung diseases.</p>



<p class="wp-block-paragraph">Pulmonary fibrosis causes scarring of the lung tissue, making the lungs stiffer and making it more difficult for oxygen to pass into the bloodstream. One of the best-known forms is <strong>idiopathic pulmonary fibrosis (IPF)</strong>, in which the exact cause of the lung scarring remains unknown.</p>



<p class="wp-block-paragraph">For someone who has recently been diagnosed with IPF, hearing the words &#8220;lung fibrosis&#8221; can be frightening. Understanding the disease, its symptoms, treatment options, and what to expect can help patients and families manage the condition more confidently.</p>



<p class="wp-block-paragraph"><strong>What Is Idiopathic Pulmonary Fibrosis?</strong></p>



<p class="wp-block-paragraph">Idiopathic pulmonary fibrosis is a chronic lung disease in which the tissue surrounding the air sacs of the lungs becomes thickened, stiff, and scarred.</p>



<p class="wp-block-paragraph">The word <strong>&#8220;idiopathic&#8221; means that the exact cause is unknown</strong>.</p>



<p class="wp-block-paragraph">As the scarring progresses, oxygen has more difficulty moving from the lungs into the bloodstream. This can gradually cause increasing breathlessness and reduced ability to perform everyday activities.</p>



<p class="wp-block-paragraph"><strong>What Are the Symptoms of IPF?</strong></p>



<p class="wp-block-paragraph">Symptoms of IPF can develop gradually and may initially be mistaken for ageing, lack of fitness, or another respiratory condition.</p>



<p class="wp-block-paragraph">Common symptoms include:</p>



<ul class="wp-block-list">
<li><strong>Shortness of breath</strong>, particularly during physical activity</li>



<li><strong>Persistent dry cough</strong></li>



<li>Increasing tiredness</li>



<li>Reduced ability to walk or exercise</li>



<li>Unintentional weight loss in some patients</li>



<li>Rapid or shallow breathing</li>



<li>Clubbing of the fingers or toes in some patients</li>
</ul>



<p class="wp-block-paragraph">The progression of IPF is not the same for everyone. Some patients may remain relatively stable for a period of time, while others may experience a more noticeable decline.</p>



<p class="wp-block-paragraph"><strong>Why Is Early Recognition Important?</strong></p>



<p class="wp-block-paragraph">Persistent breathlessness or a chronic dry cough should not simply be assumed to be due to ageing or a routine chest problem.</p>



<p class="wp-block-paragraph">IPF is one of several conditions that can cause persistent breathlessness and cough. Proper evaluation can help determine whether symptoms are caused by pulmonary fibrosis, another form of interstitial lung disease, or a different respiratory condition.</p>



<p class="wp-block-paragraph">Early recognition is also important because treatments are available that can <strong>slow the progression of IPF</strong>, even though there is currently no cure.</p>



<p class="wp-block-paragraph"><strong>Does IPF Mean the Lungs Will Keep Getting Worse?</strong></p>



<p class="wp-block-paragraph">IPF is considered a <strong>progressive lung disease</strong>, but the rate of progression can vary considerably between patients.</p>



<p class="wp-block-paragraph">Some patients remain relatively stable for a period of time, while others experience a faster decline in breathing and physical ability.</p>



<p class="wp-block-paragraph">There can also be episodes known as <strong>acute exacerbations</strong>, during which breathing suddenly becomes significantly worse. IPF can also be associated with complications such as pulmonary hypertension and respiratory failure.</p>



<p class="wp-block-paragraph">This is why regular medical follow-up and monitoring are important.</p>



<p class="wp-block-paragraph"><strong>Can IPF Be Treated?</strong></p>



<p class="wp-block-paragraph">There is currently <strong>no cure for idiopathic pulmonary fibrosis</strong>, but this does not mean that nothing can be done.</p>



<p class="wp-block-paragraph">Medicines known as <strong>antifibrotic drugs</strong>, particularly <strong>nintedanib and pirfenidone</strong>, can slow the progression of lung disease in appropriate patients.</p>



<p class="wp-block-paragraph">Depending on the patient&#8217;s condition, management may also include:</p>



<ul class="wp-block-list">
<li>Oxygen therapy when required</li>



<li>Pulmonary rehabilitation</li>



<li>Monitoring of lung function</li>



<li>Monitoring of oxygen levels</li>



<li>Management of associated medical conditions</li>



<li>Evaluation for lung transplantation in selected patients with advanced disease</li>
</ul>



<p class="wp-block-paragraph">Treatment needs to be individualised because IPF does not progress in exactly the same way in every patient.</p>



<p class="wp-block-paragraph"><strong>Does Everyone With IPF Need Oxygen?</strong></p>



<p class="wp-block-paragraph"><strong>No.</strong></p>



<p class="wp-block-paragraph">Oxygen is recommended when a patient&#8217;s oxygen level is low or when there is another clinical indication for supplemental oxygen.</p>



<p class="wp-block-paragraph">Some patients may require oxygen mainly during exercise or sleep, while others may need it for longer periods.</p>



<p class="wp-block-paragraph">The need for oxygen should be determined through appropriate assessment rather than simply because a person has been diagnosed with pulmonary fibrosis.</p>



<p class="wp-block-paragraph"><strong>Can Exercise Help Patients With IPF?</strong></p>



<p class="wp-block-paragraph">A diagnosis of IPF does not mean that a patient should stop all physical activity.</p>



<p class="wp-block-paragraph">Many patients can benefit from an appropriately designed <strong>pulmonary rehabilitation programme</strong>, which can help improve exercise capacity, physical function, and quality of life.</p>



<p class="wp-block-paragraph">The type and intensity of exercise should be appropriate for the patient&#8217;s condition, particularly if oxygen levels fall during activity.</p>



<p class="wp-block-paragraph"><strong>What Should Family Members Understand?</strong></p>



<p class="wp-block-paragraph">IPF affects not only the patient but often the entire family.</p>



<p class="wp-block-paragraph">As breathlessness progresses, patients may need assistance with activities that were previously easy, such as:</p>



<ul class="wp-block-list">
<li>Walking longer distances</li>



<li>Shopping</li>



<li>Household activities</li>



<li>Travelling</li>



<li>Attending medical appointments</li>
</ul>



<p class="wp-block-paragraph">Family members can help by understanding the disease, supporting treatment, recognising changes in symptoms, and helping the patient remain as active and independent as possible.</p>



<p class="wp-block-paragraph">Emotional support is also important. Living with a progressive lung disease can be stressful for both patients and their families.</p>



<p class="wp-block-paragraph"><strong>When Should a Patient Seek Medical Attention?</strong></p>



<p class="wp-block-paragraph">A sudden or significant worsening of breathlessness should not be ignored.</p>



<p class="wp-block-paragraph">Patients with IPF should seek prompt medical assessment if they develop:</p>



<ul class="wp-block-list">
<li>A sudden increase in breathlessness</li>



<li>A significant new or worsening cough</li>



<li>Fever or signs of infection</li>



<li>A noticeable drop in oxygen levels</li>



<li>Chest pain</li>



<li>Coughing up blood</li>



<li>Marked weakness or inability to perform usual activities</li>
</ul>



<p class="wp-block-paragraph">A sudden deterioration can have several possible causes, including infection or an acute exacerbation of IPF, and requires appropriate assessment.</p>



<p class="wp-block-paragraph"><strong>The Important Message During Pulmonary Fibrosis Awareness Month 2026</strong></p>



<p class="wp-block-paragraph"><strong>IPF is a serious lung disease, but understanding the condition is an important part of managing it.</strong></p>



<p class="wp-block-paragraph">Persistent breathlessness and a dry cough should not be ignored, particularly when symptoms are gradually worsening.</p>



<p class="wp-block-paragraph">Although there is currently no cure for IPF, modern antifibrotic treatment can slow disease progression in appropriate patients. Oxygen therapy, pulmonary rehabilitation, monitoring, and other supportive measures can also play an important role in maintaining quality of life.</p>

<p class="wp-block-paragraph">When people have a persistent cough, they often think the problem must be coming from the lungs. But <strong>acid reflux can also cause or contribute to chronic cough</strong>.</p>



<p class="wp-block-paragraph">Reflux occurs when stomach contents move back up into the esophagus. In some people, reflux can trigger coughing through irritation of the throat and esophagus or through reflux-related reflexes. Gastroesophageal reflux disease (GERD) is recognized as one of the possible causes of chronic cough.</p>



<p class="wp-block-paragraph">Importantly, <strong>not every cough is caused by reflux</strong>, and several conditions can exist together.</p>



<p class="wp-block-paragraph"><strong>How Can Reflux Cause Cough?</strong></p>



<p class="wp-block-paragraph">Reflux does not always mean that acid reaches the mouth.</p>



<p class="wp-block-paragraph">When stomach contents move upward, they can irritate the esophagus and upper airway. Reflux can also trigger a reflex between the esophagus and airways that causes coughing.</p>



<p class="wp-block-paragraph">This means a person may have a <strong>reflux-related cough even when there is no obvious acid taste or heartburn</strong>.</p>



<p class="wp-block-paragraph"><strong>Can Reflux Cause Cough Without Heartburn?</strong></p>



<p class="wp-block-paragraph"><strong>Yes.</strong></p>



<p class="wp-block-paragraph">This is one reason reflux-related cough can be difficult to recognize.</p>



<p class="wp-block-paragraph">Some people with reflux-associated cough do not experience the typical symptoms of heartburn or regurgitation. However, the absence of heartburn does <strong>not</strong> prove that reflux is causing the cough either. Other common causes of chronic cough still need to be considered.</p>



<p class="wp-block-paragraph"><strong>What Does a Reflux-Related Cough Feel Like?</strong></p>



<p class="wp-block-paragraph">There is no single type of cough that proves reflux is responsible.</p>



<p class="wp-block-paragraph">The cough may be:</p>



<ul class="wp-block-list">
<li>Dry or with some phlegm</li>



<li>Persistent or recurrent</li>



<li>Worse after meals</li>



<li>Worse when lying down in some people</li>



<li>Associated with throat irritation or frequent throat clearing</li>



<li>Associated with hoarseness or a sensation of something in the throat</li>
</ul>



<p class="wp-block-paragraph">However, these symptoms are <strong>not specific to reflux</strong>. A similar cough can occur with asthma, post-nasal drainage, smoking, COPD, bronchiectasis, certain medications, and other conditions.</p>



<p class="wp-block-paragraph"><strong>When Should Reflux Be Considered as a Cause of Cough?</strong></p>



<p class="wp-block-paragraph">Reflux becomes more relevant when a patient has a chronic cough together with symptoms such as:</p>



<ul class="wp-block-list">
<li>Frequent heartburn</li>



<li>Acid or food coming back into the mouth</li>



<li>Symptoms after meals</li>



<li>Symptoms that are worse when lying down</li>



<li>Recurrent throat irritation or hoarseness</li>
</ul>



<p class="wp-block-paragraph">CHEST guidance recommends considering reflux among the possible causes of chronic cough while also evaluating other common causes. In many patients, <strong>more than one condition may be contributing to the cough</strong>.</p>



<p class="wp-block-paragraph"><strong>Does Every Patient with Cough Need Acid-Reducing Medicine?</strong></p>



<p class="wp-block-paragraph"><strong>No.</strong></p>



<p class="wp-block-paragraph">This is an important point.</p>



<p class="wp-block-paragraph">A persistent cough should not automatically be treated with a proton pump inhibitor (PPI) simply because reflux is suspected.</p>



<p class="wp-block-paragraph">CHEST guidance recommends that in patients with suspected reflux-related cough <strong>who also have heartburn or regurgitation</strong>, treatment may include medication to control these reflux symptoms along with appropriate lifestyle measures.</p>



<p class="wp-block-paragraph">However, in patients who have a chronic cough <strong>without heartburn or regurgitation</strong>, PPI treatment alone is unlikely to resolve the cough and is not routinely recommended simply on the assumption that reflux is responsible.</p>



<p class="wp-block-paragraph"><strong>How Is Reflux-Related Cough Diagnosed?</strong></p>



<p class="wp-block-paragraph">There is no single symptom or routine test that can always prove that reflux is responsible for a cough.</p>



<p class="wp-block-paragraph">The doctor may first look for other common causes of chronic cough and assess whether the patient&#8217;s symptoms are compatible with reflux.</p>



<p class="wp-block-paragraph">In selected patients, further testing may include <strong>esophageal pH monitoring or impedance-pH monitoring</strong>, which can assess reflux episodes and their relationship with symptoms.</p>



<p class="wp-block-paragraph">Testing is particularly useful when the diagnosis is uncertain or when cough continues despite appropriate treatment.</p>



<p class="wp-block-paragraph"><strong>How Is Reflux-Related Cough Treated?</strong></p>



<p class="wp-block-paragraph">Treatment depends on whether reflux is contributing to the cough.</p>



<p class="wp-block-paragraph">Management may include:</p>



<ul class="wp-block-list">
<li>Treatment of confirmed or clinically suspected GERD</li>



<li>Appropriate acid-suppressing medication when indicated</li>



<li>Measures to reduce reflux, such as avoiding meals close to bedtime</li>



<li>Weight reduction when overweight or obese</li>



<li>Elevating the head of the bed in patients with troublesome nighttime reflux symptoms</li>
</ul>



<p class="wp-block-paragraph">It is important to understand that <strong>improvement in heartburn does not necessarily mean that the cough will immediately disappear</strong>. CHEST guidance notes that cough improvement may take considerably longer than improvement in typical reflux symptoms.</p>



<p class="wp-block-paragraph"><strong>What If My Cough Does Not Improve with Reflux Treatment?</strong></p>



<p class="wp-block-paragraph">This is an important situation.</p>



<p class="wp-block-paragraph">If the cough continues despite appropriate reflux treatment, it should <strong>not automatically be assumed that more reflux medication is needed</strong>.</p>



<p class="wp-block-paragraph">The diagnosis should be reconsidered.</p>



<p class="wp-block-paragraph">Other causes such as:</p>



<ul class="wp-block-list">
<li>Asthma</li>



<li>Upper airway cough syndrome</li>



<li>COPD</li>



<li>Bronchiectasis</li>



<li>Smoking or environmental exposure</li>



<li>Certain medications</li>



<li>Other lung diseases</li>
</ul>



<p class="wp-block-paragraph">may be responsible, either alone or together with reflux. CHEST specifically recommends considering the common causes of chronic cough rather than assuming reflux is the only explanation.</p>

<p class="wp-block-paragraph">Completing treatment for <strong>pulmonary tuberculosis (TB)</strong> is a major step toward recovery. Most people who receive an appropriate treatment regimen and complete it as prescribed are successfully cured. However, TB can occur again in some people after treatment.</p>



<p class="wp-block-paragraph">If you previously completed TB treatment and later develop symptoms such as a persistent cough, fever, night sweats, weight loss, or coughing up blood, it is important to have the symptoms properly evaluated.</p>



<p class="wp-block-paragraph"><strong>Can Pulmonary TB Come Back After Treatment?</strong></p>



<p class="wp-block-paragraph"><strong>Yes, TB can occur again after treatment.</strong></p>



<p class="wp-block-paragraph">When TB develops again after a previous episode, it may be due to:</p>



<ul class="wp-block-list">
<li><strong>Relapse</strong> – the original TB infection becomes active again.</li>



<li><strong>Reinfection</strong> – the person develops a new TB infection, potentially from a different strain of <em>Mycobacterium tuberculosis</em>.</li>
</ul>



<p class="wp-block-paragraph"><strong>Why Can TB Recur?</strong></p>



<p class="wp-block-paragraph">Recurrence can happen for different reasons.</p>



<p class="wp-block-paragraph">One important reason is that treatment was <strong>not completed adequately</strong>, allowing surviving TB bacteria to cause disease again. Missing too many doses can also increase the risk of drug resistance.</p>



<p class="wp-block-paragraph">However, recurrence does <strong>not automatically mean that a patient did something wrong</strong>. TB recurrence can occur despite appropriate treatment, and a new infection can also occur after a person has previously been successfully treated.</p>



<p class="wp-block-paragraph">Certain patients have a higher risk of recurrence, including those with extensive disease or other factors that affect treatment response. Previous TB treatment also increases concern about possible drug resistance if TB occurs again.</p>



<p class="wp-block-paragraph"><strong>How Soon Can TB Come Back?</strong></p>



<p class="wp-block-paragraph">There is no fixed time when recurrence must occur. However, TB can occur again later as well, particularly through a new infection.</p>

<p class="wp-block-paragraph">One infection that deserves particular attention is <strong>influenza, commonly called the flu</strong>.</p>



<p class="wp-block-paragraph">For someone with healthy lungs, influenza may cause fever, cough, body aches and tiredness. But in a person with chronic lung disease, the same infection can cause <strong>much more significant breathing problems</strong>.</p>



<p class="wp-block-paragraph">It can worsen existing lung symptoms, trigger an asthma attack or COPD exacerbation, and increase the risk of complications such as pneumonia and hospitalisation. People with chronic lung disease are recognised as being at higher risk of serious influenza complications.</p>



<p class="wp-block-paragraph">This is why <strong>annual flu vaccination is an important part of preventive care for many people with chronic lung disease</strong>.</p>



<p class="wp-block-paragraph"><strong>Why Is Flu More Concerning If I Already Have Lung Disease?</strong></p>

<p class="wp-block-paragraph"><strong>Yes, it is possible.</strong> The flu vaccine does not provide immediate protection—it takes about <strong>2 weeks</strong> after vaccination for protective antibodies to develop. You can therefore become infected if you are exposed to influenza before your body has developed protection.</p>



<p class="wp-block-paragraph">You may also develop <strong>flu-like symptoms from other viruses</strong> that are not influenza. The flu vaccine protects against influenza viruses, not all viruses that can cause cough, fever, sore throat or body aches.</p>



<p class="wp-block-paragraph"><strong>So, getting the flu vaccine does not mean you can never get flu-like illness—but it remains an important way to reduce your risk from influenza.</strong></p>

<p class="wp-block-paragraph">A blood test can sometimes detect a liver problem <strong>before you feel seriously unwell</strong>.</p>



<p class="wp-block-paragraph">You should also tell your doctor if you develop:</p>



<ul class="wp-block-list">
<li>Loss of appetite</li>



<li>Persistent nausea or vomiting</li>



<li>Unusual weakness</li>



<li>Abdominal discomfort</li>



<li>Dark-colored urine</li>



<li>Yellowing of the eyes or skin</li>
</ul>



<p class="wp-block-paragraph">These symptoms should not be ignored during TB treatment.</p>



<p class="wp-block-paragraph"><strong>Why Is Kidney Function Sometimes Checked?</strong></p>



<p class="wp-block-paragraph">Some TB medicines can affect the kidneys or may need adjustment if your kidney function is reduced.</p>



<p class="wp-block-paragraph">Your doctor may therefore check your kidney function, particularly if you already have kidney problems or are taking medicines that require closer monitoring.</p>



<p class="wp-block-paragraph">Again, this does not mean that you will necessarily develop a kidney problem.</p>



<p class="wp-block-paragraph">The purpose of monitoring is to <strong>identify problems early rather than waiting until they become serious</strong>.</p>



<p class="wp-block-paragraph"><strong>Why Might I Need a Vision Test?</strong></p>



<p class="wp-block-paragraph">If your TB treatment includes <strong>ethambutol</strong>, your doctor may check your vision.</p>



<p class="wp-block-paragraph">This is because ethambutol can rarely affect the optic nerve.</p>



<p class="wp-block-paragraph">You should tell your doctor promptly if you notice:</p>



<ul class="wp-block-list">
<li>Blurred vision</li>



<li>Difficulty seeing</li>



<li>A change in color vision</li>



<li>Any new or unusual visual problem</li>
</ul>

<p class="wp-block-paragraph"><strong>What Can You Do as a Patient?</strong></p>



<p class="wp-block-paragraph">You can make your TB treatment safer by following a few simple rules:</p>

<p class="wp-block-paragraph">When a doctor tells someone, <strong>&#8220;You have COPD,&#8221;</strong> one of the first questions is often:</p>



<p class="wp-block-paragraph"><strong>&#8220;But I have never smoked. How can I have COPD?&#8221;</strong></p>



<p class="wp-block-paragraph">It is a reasonable question.</p>



<p class="wp-block-paragraph">Smoking is the most important individual risk factor for COPD in many populations, but <strong>smoking is not the only way COPD can develop</strong>.</p>



<p class="wp-block-paragraph">The 2026 GOLD report recognises that COPD can result from exposure to harmful particles and gases other than tobacco smoke, as well as factors affecting lung development and ageing. GOLD notes that non-tobacco risk factors account for a substantial proportion of COPD worldwide.</p>



<p class="wp-block-paragraph">Therefore, <strong>being a lifelong non-smoker does not rule out COPD</strong>.</p>



<p class="wp-block-paragraph">However, if you have never smoked, your diagnosis deserves careful evaluation. It is important to confirm that you actually have persistent airflow obstruction and to look for the factors that may have contributed to your lung disease.</p>



<p class="wp-block-paragraph"><strong>What Is COPD?</strong></p>



<p class="wp-block-paragraph"><strong>Chronic obstructive pulmonary disease (COPD)</strong> is a chronic respiratory condition associated with persistent respiratory symptoms and airflow limitation caused by abnormalities of the airways and/or air sacs of the lungs.</p>



<p class="wp-block-paragraph">Common symptoms include:</p>



<ul class="wp-block-list">
<li>Shortness of breath</li>



<li>Chronic cough</li>



<li>Phlegm or mucus production</li>



<li>Wheezing</li>



<li>Reduced exercise capacity</li>



<li>Recurrent respiratory symptoms</li>
</ul>



<p class="wp-block-paragraph">COPD is not one identical disease in every patient. Some people have predominantly airway disease, while others have emphysema or a combination of abnormalities.</p>



<p class="wp-block-paragraph"><strong>Can You Really Have COPD Without Smoking?</strong></p>



<p class="wp-block-paragraph"><strong>Yes.</strong></p>



<p class="wp-block-paragraph">The 2026 GOLD report specifically recognises several non-smoking factors that can contribute to COPD.</p>



<p class="wp-block-paragraph">These include:</p>



<ul class="wp-block-list">
<li>Household air pollution and biomass smoke</li>



<li>Occupational exposure to dust, fumes, and chemicals</li>



<li>Secondhand smoke</li>



<li>Outdoor air pollution</li>



<li>Abnormal lung development</li>



<li>Genetic factors</li>



<li>Other long-term exposures to harmful particles and gases</li>
</ul>



<p class="wp-block-paragraph">GOLD also highlights that the relative importance of these factors differs between countries and populations. Non-smoking causes are particularly important in many low- and middle-income countries.</p>



<p class="wp-block-paragraph">So if you have never smoked, the next question should be:</p>



<p class="wp-block-paragraph"><strong>&#8220;What other factors may have affected my lungs?&#8221;</strong></p>



<p class="wp-block-paragraph"><strong>1. Biomass Smoke Can Damage the Lungs</strong></p>



<p class="wp-block-paragraph">One of the most important causes of COPD in non-smokers is <strong>long-term exposure to household air pollution</strong>.</p>



<p class="wp-block-paragraph">Burning fuels such as:</p>



<ul class="wp-block-list">
<li>Wood</li>



<li>Animal dung</li>



<li>Crop residues</li>



<li>Coal</li>
</ul>



<p class="wp-block-paragraph">can produce large amounts of harmful particles and gases, particularly when cooking or heating takes place in poorly ventilated areas.</p>



<p class="wp-block-paragraph">This is particularly relevant in developing countries, where household biomass exposure remains common. GOLD identifies household air pollution as an important risk factor for COPD.</p>



<p class="wp-block-paragraph">Someone who has never smoked may therefore have had substantial exposure to smoke throughout their life without considering themselves a &#8220;smoker.&#8221;</p>



<p class="wp-block-paragraph"><strong>2. Your Workplace May Have Affected Your Lungs</strong></p>



<p class="wp-block-paragraph">Occupational exposure is another important consideration.</p>



<p class="wp-block-paragraph">Long-term exposure to:</p>



<ul class="wp-block-list">
<li>Dust</li>



<li>Chemical fumes</li>



<li>Industrial particles</li>



<li>Construction dust</li>



<li>Welding fumes</li>



<li>Agricultural dust</li>



<li>Other workplace pollutants</li>
</ul>



<p class="wp-block-paragraph">can contribute to chronic lung damage.</p>



<p class="wp-block-paragraph">GOLD identifies occupational exposure to organic and inorganic dusts, chemical agents, and fumes as an under-recognized COPD risk factor. The impact can be particularly important among people who have never smoked.</p>



<p class="wp-block-paragraph">When evaluating COPD in a non-smoker, your doctor may therefore ask about your <strong>entire working history</strong>, not just your smoking history.</p>



<p class="wp-block-paragraph"><strong>3. Secondhand Smoke Also Matters</strong></p>



<p class="wp-block-paragraph">You may never have smoked yourself but may have spent years around people who smoked.</p>



<p class="wp-block-paragraph">Long-term exposure to secondhand tobacco smoke can increase respiratory risk and may contribute to COPD.</p>



<p class="wp-block-paragraph">This can be especially relevant if exposure occurred at home or in the workplace over many years.</p>



<p class="wp-block-paragraph"><strong>4. Air Pollution May Contribute</strong></p>



<p class="wp-block-paragraph">Outdoor air pollution exposes the lungs to harmful particles and gases.</p>



<p class="wp-block-paragraph">Traffic emissions, industrial pollution and particulate matter can contribute to respiratory disease.</p>



<p class="wp-block-paragraph">The relationship between outdoor air pollution and COPD is complex, and it may not explain COPD by itself in an individual patient. However, GOLD recognises inhalation of toxic particles and gases from outdoor air pollution as part of the overall environmental exposure that can contribute to COPD.</p>



<p class="wp-block-paragraph"><strong>5. Your Lungs May Not Have Reached Their Full Potential</strong></p>



<p class="wp-block-paragraph">COPD is not always simply the result of lung damage occurring later in life.</p>



<p class="wp-block-paragraph"><strong>How well your lungs developed during childhood and early life can also matter.</strong></p>



<p class="wp-block-paragraph">Factors affecting lung development may include:</p>



<ul class="wp-block-list">
<li>Premature birth</li>



<li>Poor lung growth</li>



<li>Low birth weight</li>



<li>Childhood respiratory problems</li>



<li>Recurrent severe respiratory infections</li>



<li>Exposure to smoke and pollution during early life</li>
</ul>



<p class="wp-block-paragraph">GOLD recognises abnormal lung development and impaired lung growth as important host factors that can increase vulnerability to COPD later in life.</p>



<p class="wp-block-paragraph"><strong>6. A Genetic Condition May Be Responsible</strong></p>



<p class="wp-block-paragraph">One genetic condition that doctors particularly consider is <strong>alpha-1 antitrypsin deficiency</strong>.</p>



<p class="wp-block-paragraph">Alpha-1 antitrypsin is a protein that helps protect lung tissue.</p>



<p class="wp-block-paragraph">People with severe deficiency can develop emphysema at a younger age, sometimes despite having never smoked.</p>



<p class="wp-block-paragraph">This is one reason that genetic causes should be considered when COPD occurs at a relatively young age, particularly when there is little or no smoking history.</p>



<p class="wp-block-paragraph"><strong>7. Previous Lung Disease May Be Part of the Story</strong></p>



<p class="wp-block-paragraph">Some people develop chronic airflow limitation after previous respiratory disease or other damage to the lungs.</p>



<p class="wp-block-paragraph">For example, severe respiratory infections during childhood can affect subsequent lung development and function.</p>



<p class="wp-block-paragraph">Previous <strong>tuberculosis</strong> can also leave permanent structural abnormalities in the lungs and may be associated with chronic airflow limitation.</p>



<p class="wp-block-paragraph">However, this does <strong>not</strong> mean that everyone who has had tuberculosis or a severe chest infection has COPD.</p>



<p class="wp-block-paragraph">The actual pattern of lung damage needs to be evaluated.</p>



<p class="wp-block-paragraph"><strong>But Are You Sure It Is COPD?</strong></p>



<p class="wp-block-paragraph">This is particularly important if you have never smoked.</p>



<p class="wp-block-paragraph"><strong>Breathlessness, cough, wheezing and phlegm do not automatically mean COPD.</strong></p>



<p class="wp-block-paragraph">Several other conditions can cause similar symptoms, including:</p>



<ul class="wp-block-list">
<li>Asthma</li>



<li>Bronchiectasis</li>



<li>Previous tuberculosis or other lung infections</li>



<li>Interstitial lung disease</li>



<li>Heart disease</li>



<li>Pulmonary hypertension</li>



<li>Other respiratory conditions</li>
</ul>



<p class="wp-block-paragraph">This is why the diagnosis should not be based on symptoms alone.</p>



<p class="wp-block-paragraph"><strong>Spirometry Is Essential</strong></p>



<p class="wp-block-paragraph"><strong>Spirometry is the key test used to confirm persistent airflow obstruction in COPD.</strong></p>



<p class="wp-block-paragraph">The 2026 GOLD report continues to emphasise the importance of spirometry, and in March 2026 GOLD and the Global Lung Function Initiative (GLI) issued a joint statement specifically addressing how spirometry should be interpreted when confirming COPD.</p>



<p class="wp-block-paragraph">If you have been told that you have COPD despite never smoking, it is reasonable to ask:</p>



<p class="wp-block-paragraph"><strong>&#8220;Was my diagnosis confirmed with properly performed spirometry?&#8221;</strong></p>



<p class="wp-block-paragraph">Depending on your symptoms and test results, your doctor may also recommend additional pulmonary function testing or imaging.</p>



<p class="wp-block-paragraph"><strong>Why Does the Cause Matter?</strong></p>



<p class="wp-block-paragraph">Finding out why you have COPD is not simply about giving the disease a name.</p>



<p class="wp-block-paragraph">It can help your doctor identify exposures that should be reduced or avoided and determine whether another condition may be contributing to your symptoms.</p>



<p class="wp-block-paragraph">For example:</p>



<ul class="wp-block-list">
<li>Occupational exposure may need to be addressed.</li>



<li>Biomass smoke exposure should be reduced.</li>



<li>Alpha-1 antitrypsin deficiency may require specific evaluation.</li>



<li>Previous tuberculosis may have produced structural lung damage.</li>



<li>Asthma or bronchiectasis may require a different treatment approach.</li>
</ul>



<p class="wp-block-paragraph"><strong>The correct diagnosis leads to the correct treatment.</strong></p>



<p class="wp-block-paragraph"><strong>Do I Need COPD Inhalers If I Never Smoked?</strong></p>



<p class="wp-block-paragraph">Possibly.</p>



<p class="wp-block-paragraph">The decision to use inhalers is <strong>not based on whether you have smoked</strong>.</p>



<p class="wp-block-paragraph">Treatment is based on your confirmed diagnosis, symptoms, exacerbation history, lung function, and individual risk factors.</p>



<p class="wp-block-paragraph">If COPD is confirmed and inhaled treatment is appropriate, your doctor may prescribe bronchodilator therapy or other medication according to your clinical situation.</p>



<p class="wp-block-paragraph">But if the underlying problem is actually asthma, bronchiectasis, or another condition, the treatment plan may be different.</p>



<p class="wp-block-paragraph">This is another reason why confirming the diagnosis is so important.</p>



<p class="wp-block-paragraph"><strong>What Can I Do If I Have COPD and Never Smoked?</strong></p>



<p class="wp-block-paragraph">If COPD has been confirmed, several measures can help protect your respiratory health.</p>



<p class="wp-block-paragraph"><strong>Avoid further exposure</strong></p>



<p class="wp-block-paragraph">Try to minimise exposure to:</p>



<ul class="wp-block-list">
<li>Tobacco smoke</li>



<li>Biomass smoke</li>



<li>Dust</li>



<li>Chemical fumes</li>



<li>Workplace pollutants</li>



<li>Other known respiratory irritants</li>
</ul>



<p class="wp-block-paragraph"><strong>Use prescribed treatment correctly</strong></p>



<p class="wp-block-paragraph">If inhalers have been prescribed, use them according to your doctor&#8217;s instructions and make sure your inhaler technique is correct.</p>



<p class="wp-block-paragraph"><strong>Stay physically active</strong></p>



<p class="wp-block-paragraph">Appropriate physical activity and pulmonary rehabilitation can help improve exercise capacity and reduce the impact of breathlessness in suitable patients.</p>



<p class="wp-block-paragraph"><strong>Prevent respiratory infections</strong></p>



<p class="wp-block-paragraph">Vaccination and appropriate preventive measures are important components of COPD care.</p>



<p class="wp-block-paragraph"><strong>Attend regular follow-up</strong></p>



<p class="wp-block-paragraph">COPD can change over time. Regular assessment allows your doctor to review symptoms, exacerbations, inhaler technique, lung function, and other health problems.</p>

<p class="wp-block-paragraph">For family members and caregivers, caring for someone with advanced COPD can also be challenging. You may find yourself helping with medicines, oxygen, appointments, household activities, meals, and watching for changes in breathing.</p>



<p class="wp-block-paragraph">The goal of care is not simply to treat the lungs. It is to help the person <strong>breathe as comfortably as possible, remain as independent as possible, avoid unnecessary flare-ups and hospital visits, and maintain the best possible quality of life</strong>.</p>



<p class="wp-block-paragraph"><strong>What Does Advanced COPD Mean?</strong></p>



<p class="wp-block-paragraph">Advanced COPD generally refers to COPD that has become severe enough to significantly affect breathing, physical activity, independence, and quality of life.</p>
